What scale does while you're not looking
Hard water minerals drop out of solution when water gets heated or evaporates, and they leave a chalky white crust. In a dishwasher that crust coats the spray arm holes and the heating element. In a water heater it settles on the bottom of the tank as a gritty layer. Washing machine valves and hoses collect it, and ice makers get it in the fill tube. None of this breaks anything on day one. It just makes every part work harder. Heating elements run longer to hit temperature, pumps push against narrower openings, and parts that should last years start failing early.
The habit that actually prevents it
Descale on a schedule instead of waiting for symptoms. For the dishwasher, run an empty hot cycle with a cup of white vinegar in a bowl on the top rack, or use a citric acid dishwasher cleaner made for the job. Coffee maker, same idea. For the washing machine, run a hot clean cycle with a machine cleaner tablet, and wipe the door gasket and detergent drawer while you're in there. If you have a water softener, check the salt level monthly. Empty softener, no protection. A whole-home filter or softener does more than any cleaning cycle can.
How often is often enough
In Lubbock and most of the Panhandle, water is hard enough that monthly is a reasonable target for the dishwasher and coffee maker. Every three months for the washing machine. Pull and rinse the dishwasher filter monthly under the tap with a soft brush. Check the refrigerator water filter and change it on the date the manual gives you, sooner if flow slows. Water heaters need a drain and flush once a year. If you haven't done it in five years, have someone do it rather than guessing. Softened water lets you stretch these intervals somewhat, not skip them.
Signs prevention time has passed
Glasses that come out gritty or cloudy even after a vinegar cycle mean the spray arms are partly plugged. Dishes still wet at the end of the cycle can point to a heating element buried in scale. A water heater that pops, rumbles, or bangs when it fires has sediment sitting on the bottom, and hot water that runs out sooner than it used to is the same story. On a washer, cycles that take much longer to fill point to restricted valves. An ice maker making small, hollow, or slow cubes has a mineral problem in the line. Those need hands on them, not a cleaning tablet.

Occasional use in an empty cycle is fine for most machines. Don't leave vinegar sitting in the tub for hours, and check your manual, since a few makers prefer a citric acid cleaner on rubber seals.
Does a water softener mean I can stop descaling?
It cuts the mineral load a lot, so you can space cleanings further apart. You still have to run the salt down and refill it, clean the dishwasher filter, and flush the water heater yearly.
Can scale damage be reversed once it's built up?
